We require  that a Release of Liability form be completed and signed by the participant for each training, even if we have trained your dog in the past.

The 2-page form includes questions about your dog including a pertinent medical history which is very important so that the  training is performed with your dog's health in mind.  There may be instances where we are unable to train your dog due to a recent medical event condition, such as a recent surgery or heart condition.

Before we train your dog, we will verbally go through the form questions to cover your dog's  specific needs.
